Job Profile:

The Case Manager is dedicated to offering intensive case management for seniors aged 65 and older with a serious mental illness and age related issues. The team works towards building partnerships and developing support networks for clients and their care givers. This program will help to increase the supports seniors are already receiving from their current support systems and will assist them to stay in their homes longer and help to increase their quality of life and well-being. Together, a support plan is prepared, addressing areas such as primary healthcare, and access issues for housing and/or income; we assist clients to engage in their local community to reduce their experience of social isolation. Seniors will be provided with service coordination and advocacy.

 

The Case Manager works with clients to provide individualized and flexible services to engage, support and integrate clients into the community, and facilitate their use of resources and supports to maximize their independence and potential. Using an Empowerment/Recovery approach, the Psychogeriatric Case Manager provides support, assistance, referrals, and education to clients, and with their permission, their families.

 

In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.

 

The Case Manager may be assigned to different programs as required based on need. (i.e. ACTT, Early Psychosis Intervention, Case Management).

 

This position reports to and is supervised by the Program Manager.

 

Requirements:

  • Degree in health or social services plus minimum four years or Diploma in health or social services plus minimum six years relevant experience in a community-based organization working with senior adults with serious mental illness
  • Post-secondary education in Gerontology is an asset
  • Current registration with the OCSWSSW or a regulatory body under the Regulated Health Professions Act
  • A second language (French, Cantonese, Mandarin, Russian, Italian, Persian, Punjabi, Korean, Tamil, Urdu) reflecting the local community is preferred.
  • Experience working with individuals experiencing serious mental illness, concurrent disorders and dual diagnosis (developmental disability and/or traumatic brain injury) is preferred
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPPA requirements.
  • Knowledge of PSR and Recovery as guiding principles.
  • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with clients.
  • Extensive knowledge of supports and services in York Region and South Simcoe, including formal and informal resources.
  • Knowledge of systemic issues such as poverty, unemployment, stigma, medications and the isolation felt by individuals with serious mental health issues and their families.
  • A valid Canadian driver’s license, a minimum $1,000,000 third party liability insurance and proof that insurance covers use of personal vehicle to transport clients
  • A satisfactory Vulnerable Sector Screening (Police Check)

 

Working Conditions:

Office work, community settings, meetings in clients home or on the phone and travel with clients
